Please arrange to fill the following template and directly email to directly to Kevin and Donnie at JF Petroleum. Once they receive the forms they are going to coordinate with you directly in order to fill out the main application for the program and any other details which they would like to have from you. The deadline for emailing the following template is the end of the business 10/28/2021.
